Last year I found myself seeking feedback from the teams I worked with in honor of continuous personal improvement so I put together my own personal  360-degree evaluation, based on what mattered to me and sent out a web form (I used Google Forms) seeking anonymous responses.

Here are the questions I asked, in case you are in a similar position.
  1. Does (name) communicate clearly in writing and verbally?
  2. When you think of effective leadership, what comes to mind?
  3. Does (name) display those traits?
  4. Would you describe (name) as a servant-leader? “A servant-leader focuses primarily on the growth and well-being of people and the communities to which they belong. While traditional leadership generally involves the accumulation and exercise of power by one at the top of the pyramid, a servant leadership is different. The servant-leader shares power, puts the needs of others first and helps people develop and perform as high.” – Robert K. Greenleaf Center for Servant Leadership
  5. Does (name) create an environment of psychological safety for the team? According to studies done at Google (see Project Aristotle), psychological safety is the most important factor in high-performing teams.
  6. Do you trust that (name) serves the team and the organization with the best of intentions and with integrity?
  7. Do you consider (name) to be a strong facilitator?
  8. What do you see as (name)’s superpowers or strongest skills?
  9. What suggestions do you have for (name) that would make her a more effective contributor to the team?
  10. Are (name)’s work methods and approach to accomplishing her job effective, efficient, and continuously improving?
  11. Would you describe (name) as a creative problem solver?
  12. Would you describe (name) as someone who has a growth mindset? According to Carol Dweck, “In a growth mindset, people believe that their most basic abilities can be developed through dedication and hard work‚ and talent are just the starting point. This view creates a love of learning and a resilience that is essential for great accomplishment.”
  13. Do you receive everything you need from (name) as a project manager in order to be successful on the project? (Tools, processes, communication, support…)
  14. Do you feel the project team has autonomy on the projects (name) manages?
  15. Do you feel that (name) values your contributions to the team?
  16. Does (name) encourage the team to take ownership of the project?
  17. Do you feel positively or negatively impacted by working with (name)?
  18. If you could choose your team for your next project, would you choose to work with (name) again?
  19. Why or why not?
  20. Do you have anything else to share regarding your experiences working with (name)?
